Abstract: Question:
Neurorehabilitation has a major role in treating neurological disorders and language impairment with traumatic or congenital origin. Neuroplasticity is subject of current research and can be induced by training[2]. Based on that, the Padovan-Method® Neurofunctional Reorganization is a promising approach in neurorehabilitative speech therapy. Its use is based on a long-time experience of certified therapists, but official treatment recommendations are still lacking. The development of a consensus-based treatment recommendation for the use of this method is the aim of the current project.
Methods:
A two-step project was set up:
(1) A systematic literature search for the term padovan[title] OR padovan[abstract] using relevant databases added by hand search and offline sources. The term was adapted, where necessary, for each database.
(2) An online expert inquiry (Delphi-process) is carried out providing at least three question rounds including German-speaking therapists with at least four out of five training modules of the Padovan-Method®. The questions for round No 1 are evaluated in a pretest. The results will be analyzed qualitatively by three independent assessors using MAXQDA. Finally, a treatment recommendation will be developed according to the consensus principle (ESPEN).
Results:
(1) The search was carried out from July to September 2019. 9 studies and 17 case reports were identified. Data extraction is in progress and the manuscript in preparation.
(2) A pretest for round No 1 was carried out from July to August 2019 (n=10). Results were implemented into the following round. Experts (n=117) were invited for round No 1.
A response rate of 49 % (n=57 ) was achieved during 5 weeks from August to September 2019. Results are currently under revision using MAXQDA to prepare the questionnaire for round No 2. Further results are expected by mid-2020.
Conclusion:
A feasible and evidence-based approach to develop a treatment recommendation and a scientific framework for the Padovan-Method® in neurorehabilitation using the Delphi-process to access and analyze expert knowledge is hereby presented. The outcome will provide indicating diagnoses and expectancy levels for prescribing physicians. Consequently, the identified indications and recommendations will have to be evaluated regarding safety, effectiveness and economic parameters in appropriate follow-up projects.
